Another Classic Michigan Model Of Stealin' - StoneCrest

StoneCrest Center

Why stop at Stonecrest?

Stonecrest is a classic, textbook example of a Corporate Shape Shifter.

Why not ask the privateers who issue "Marques of Quality Care"?

As seen here, in LARA, this operation would always slap a new sign on front of the building all the time.

When a youth would file one of those Recipient Rights Complaints, it was always tossed.

I know.

I used to talk to them.

Then I would file a grievance with Michigan, strictly for documentational purposes, only, because all they did was toss, or send me a gratuitous phone call, or dismissive missive, letting me know everything was status quo.

See, there are no civil rights in child welfare because it is all privatized under foreign corporate shape shifters who like to use LLCs because it is easy to dissolve, or bankrupt, or sell, gift, trust or something, right after you get the state grants. contracts and the fake ass Medicaid cost reimbursements because, more than likely, they took out some mortgages, DLA Small Business loan, TARP, then quiet titled, then mortgaged again, or some stealin' like that.

Let me check right quickly.

All they ever want is to turn a profit.

How is it one can turn a profit when engaging in the healing of a child?

Medicaid Fraud in Child Welfare, of course.

Oh, there are so many fraudulent billing scams that go on in child welfare.

I used to let them take me in their offices and show me everything, including voicing their grievances that Michigan will not do a damn thing for these children.

Many of the staff should not even be in the field of human services, ever, seriously, there should be some form of ethics involved when it comes to hiring practices in human resources... when it comes to human asset management for foreign, privatized, entities.

Remember, there are no civil rights in child welfare because it is privatized, where the foreign entity comes up with its own, self-reporting, of all the wonderful things they do for kids.

State didn't investigate psych patient deaths after release from Detroit hospital

Lawmakers call for more review of post-release deaths

Notification of Death Report
LANSING (WXYZ) — Since 2017, at least four patients have died within two days of being released from StoneCrest Hospital, a private inpatient psychiatric facility in Detroit. Despite being notified of each of the deaths, state officials never investigated.

Officials with LARA, the Department of Licensing and Regulatory Affairs, say they’re not legally required to follow up on deaths like these, nor are they empowered to by statute. Today, in response to an investigation by 7 Action News, two state senators are calling for changes to ensure that post-release deaths are investigated.

RELATED: A Detroit psychiatric hospital released him Thursday, he took his life Friday

At 162 beds, StoneCrest is one of the largest private psychiatric facilities in the state. 7 Action News first investigated the hospital earlier this year, after one of its patients, Joe Sadlak, admitted to murder only two days after he was discharged.

He had been treated at StoneCrest for less than a week.

“You put my son on four different kinds of medication and knew his drug history,” said Pamela Rowley, Joe’s mother, “and you let him go.”

In Michigan, when a psychiatric patient dies within two days of being released from a hospital, the state requires the hospital to report it. Those reports come to the Department of Licensing and Regulatory Affairs.

In September 2017, StoneCrest reported that a patient came to its hospital after stating he “was not happy in his life and wanted to die.” After treatment, he was released and died shortly thereafter. StoneCrest reported the death to the state, listing the cause as “not known.”

Less than a month later, another patient came to Stonecrest after saying he wanted to “jump off a bridge.” Within 2 days of release, he was found dead in his bathtub with his throat cut. State officials from LARA didn’t investigate.

A third death would be reported the following July. A woman came to Stonecrest with a history of suicidal behavior. She was treated, released and—the next day—found dead in a river. The cause of death was listed as “unknown.” Again, state officials didn’t investigate.

Then, this past June, a 29-year-old Detroit man came to Stonecrest after displaying severe psychotic behavior. He was deemed well enough to be released, but took his life the next day.

“Did your office ask any questions about that?” asked Channel 7’s Ross Jones.

“Again, we’ve been doing our statutory requirement,” said Larry Horvath, the Director of the Bureau of Community and Health System at LARA

“Did you ask any questions about that?” Jones asked again.

“We required the report to be filled out,” Horvath said.

“And that’s it,” Jones replied.

“Yes,” Horvath said.

Officials asked no questions, according to a spokesman, because “LARA is not by law or rule required to follow up on these reported deaths,” nor—they say— does the law give them the express authority to. State watchdogs are empowered to investigate only deaths that occur in restraints, Horvath said.

The death reports are reviewed during regular inspections, but no investigations are launched. Horvath said he hopes hospitals use the reports to guide their treatment.

"If they're starting to notice a pattern with a cause of death that occurs have discharge, hopefully they're starting to work with their medical directors, their nursing staff, their social workers to put corrections in place to address it," he said.

“It’s inexcusable,” said Mark Reinstein, President and CEO of the Mental Health Association in Michigan. “To hide behind, ‘It’s not our legal responsibility.’ Well then whose is it?”

By law, LARA is required to report all psychiatric deaths reported to them to the state legislature. But they haven’t been, 7 Action News has learned, for at least the last three years. In August, prompted by our reporting, the state submitted reports for 2016, 2017 and 2018, though they did not include post-release deaths.

Following our questions, LARA changed the reporting requirements for post-release deaths—no longer requiring that deaths occurring within 48 hours of discharge be reported. Spokesman Jason Moon said changing the policy aligns LARA with state law, which doesn’t require notification of post-release deaths.

The decision stunned multiple mental health advocates, including Andrea Rizor, the Director of Advocacy for Michigan Protection and Advocacy Services,.

“It looks like they don’t want to know if it’s a suspicious death or not,” she said. "It doesn't make sense."

This week, in response to 7 Action News' findings, the Democratic leader in the state senate is calling for changes.

“Stories like these are deeply concerning, and as a state we should be doing all that we can to help stop these preventable deaths,” said Senate Minority Leader Jim Ananich (D-Flint). “The department should have the ability to identify concerning patterns, and if it requires a legislative fix to make sure they are empowered to do so, we will write the bill.”

Michigan Foster Care Cover Up Death Squad Busted Again: From Ricky Holland To Omarion Humphrey - Not A Got Damn Thing Has Changed - Just Ask Nancy Edmunds

Ricky Holland
Word in the chatterverse has it that the autopsy says that he did not drown.

Michigan has always had a clean up squad in child welfare.

He should never have been there.

Ricky Holland should never have died, either.

Go get that piece of shit Steve Yager, now.

Go get Orlene Hawks.

Go get that entire Office of Children's Ombudsman.

Go get that Michigan Children's Institute.

Go get The Madame Maura Corrigan (said in a high British accent).

I command thee.

Foster agency connected to autistic boy who drowned agrees to close

BURTON, MI -
Omarion Humphrey
The foster care agency connected to the disappearance and drowning of a severely autistic boy has closed under an agreement with the Michigan Department of Health and Human Services.

Alternatives Services for Children and Families voluntarily surrendered its license and closed on Monday, July 1, 2016, according to an agreement with MDHHS Executive Director Steve Yager.

A state investigation into 9-year-old Omarion Humphrey's death recommended the license surrender. Investigators found that Alternative Services for Children and Families' license should be revoked because of licensing violations at multiple foster homes, MDHHS Spokesman Bob Wheaton said.

Humphrey was visiting Lake Callis in Davison Township with his foster family on July 4, 2015, when he disappeared. Six days later, following a massive search, his body was found in the lake. He apparently drowned.

Body in lake believed to be missing autistic boy, police said.

DAVISON TWP, MI - A body found today in Lake Callis is believed to be that of missing 9-year-old boy with autism, Omarion Humphrey, police said. Rescue crews have found a child's body in the water in Lake Callis in Davison Township....

Last year, the state verified that the 95 children under Alternative Services for Children and Families were safe and began transferring those cases to other private agencies and to MDHHS foster care workers.

Alternative Services for Children and Families still had about 40 children under its care as of June, but all cases were transferred by July 1, Wheaton said.

"Protecting vulnerable children is our top priority," Yager said. "We kept that in mind in recommending revocation of the license and in handling the closing. We took numerous steps to make sure that all children and foster families received appropriate services during this transition."

Revocation of a foster care agency license is rare and occurs when there are serious licensing violations that make closure necessary to protect children, the state said. Alternatives worked collaboratively with the department in reaching an agreement to surrender its license, according to a statement.

The Flint Journal-MLive.com could not reach a representative for Alternatives for Children and Families for comment.

An August report from the MDHHS initially recommended the license revocation saying Alternatives for Children and Families had placed a child, presumably Omarion, in a foster home that had several previous infractions, many for lack of supervision.

Agency that placed autistic boy who drowned should lose license, state says
The state says the foster agency that placed 9-year-old Omarion Humphrey, the severely autistic boy who went missing and drowned in a Davison Township park, should lose its license.

The special investigation report, carried out in response to Omarion's disappearance and death, didn't identify Omarion's foster home or refer to Omarion by name. It refers to a "Foster Child A" who went missing.

The report also refers to a Foster Parent A who did not receive training on autism. The same Foster Parent A, according to the report, took in a child on the same day court documents show Omarion was removed from his biological home in Oakland County.

Missing autistic boy pulled from home squalor just months ago, court says
Investigators said Omarion was sleeping on the floor in a trash-ridden home without adult supervision, wore dirty clothes that didn't fit and had poor hygiene, the court documents say.

"Per the rule, Foster Parent A should have been provided training that related to Foster Child A's needs," the August report said. "At the very least, Foster Parent A should have been trained on how to handle issues dealing with Foster Child A's communication skills, physical safety and self-care."

In an Oakland County court hearing last July, a representative of Alternatives testified that Omarion's foster mother was licensed to foster special needs children, but did not have formal training for managing children with autism.

The foster mother used Internet research to educate herself on caring for autistic children, the Alternatives representative said.

Omarion and 10 of his siblings were removed from their biological parents' home and placed in foster care in 2015. Investigators said Omarion was sleeping on the floor in a trash-ridden home without adult supervision, wore dirty clothes that didn't fit and had poor hygiene, the court documents say.

A lawsuit that Omarion's estate has filed against Alternatives and foster mother Lulu Richardson is pending in Genesee Circuit Court.

Michigan Child Welfare Administration: Same Day, Same Administrators

I bet this just sucks, well, I mean with what I am about to say about preserving incestuous institutions of child welfare administration of Michigan.

The DHHS Director, Nick Lyons is not in the best mental condition to even be executing orders in transfer of powers to the new Children's Services Agency Director, Dr. Herman McCall, formerly Juvenile Justice Program Director from this failure of an ethical creature Steve Yager (this is strictly a personal assessment in his professional capacity, in supposition to his willfully nefarious actions, just for his own personal pleasure of covering up federal fraud...or he is really just a dumb ass.  Either way, I am justified in both positions.)

For those of you who are unfamiliar with McCall's background in running juvenile justice programs, allow me to take you on a stroll back down memory lane.

Michigan Moves To Close Kiddie Jails

So, as rumor has it, Michigan has a policy where the emails of a former administrator are wiped from the server and hidden from any access through FOIA.

This means that, unless someone is willing to hand over their first born child in the form of outrageous FOIA charges to secure the emails, all evidence of fraudulent activities are shredded into the black hole of Michigan secretive recordkeeping in child welfare.

And, as another rumor has it, Michigan is about to launch its new model to warehouse its child welfare population, or, as they are going to spin it, foster care reforms are going to take on the model of group homes.

That is correct.  Foster care in Michigan is about to be transformed into mini-plantations, where children, who are removed from homes, mainly for being poor, are going to be placed in privatized group homes, where there is little, if any, enforceable oversight because the Bureau of Child and Adult Licensing has been stripped of any of its oversight powers due to issues of immunizing the state.

If my term of mini-plantations comes across as alleging chicanery in state administrative operations, then, how about applying the term micro-residential institutions?

Think of it like this:

Michigan is about to re-interpret what is called "home-based/community-based services" under Medicaid to generate more creative layers of service billing which does nothing to promote the viability of families in need, but it does maximize revenue for these contractual micro-residential institutions which will have no incentive of ameliorating the situations which brought children into care, in the first place, or rather a continuance of human trafficking.

This would lead to a justification of shuttering these out dated juvenile justice facilities, which would not be a bad thing, if only there existed some form of oversight in the secret operations of child welfare in Michigan.

Until which time, Michigan will carry on with its horrific treatment of children in child welfare and remain under federal monitoring through its incestuous preservation of child welfare administration through recycling its experts in covering up fraud.

Same day, same administrators.

Michigan Child Welfare Fraud Cover Up Artist Steve Yager Is Finally Leaving

Michigan Privateer Daire Rendon,
Silencer of "The Poors"
Well, this is interesting.

I recently blasted the Michigan House Standing Committee Chair, Daire Rendon, on her timing of posting public notice of the committee meetings.

Then, on March 9, 2017, there was a 21 hour advanced notice of a meeting that was, as only one can assume, was cancelled, considering there was no cancellation notice.

So, to just put a spoiler alert to the rescheduled meeting, allow me to present the rumored retirement of the most "out-cold blooded" master of cover ups in the State of Michigan in dealing with child welfare fraud, the one, the only, Steve Yager.

Rumor has it Steve Yager, and his legacy are out of here.

So, instead of allowing Steve Yager to go out a hero, I am just going to remind my fans of some of the major milestones which led up to the State being under a court monitor for the way it operates, which I consider nothing more than a criminal operation for federal billing of human trafficking, of which, no one will ever speak upon in a public Standing Committee on Families, Children and Seniors.

Let us just sit back and see if Lil' Miss Rendon will ever be considerate to public input of her Standing Committee Meeting of Families, Children, and Seniors by publishing the minutes of the meeting to announce the next scheduled meeting so citizens may provide testimony into the public record and have enough time to coordinate their calendars and budgets to get up to Lansing.

Her term is young, and ripe for the picking on.

Nominate Debbie Williams For The Michigan Child Welfare Advocate Of The Year

Each year, in Michigan, this little group of sanctimonious arbitrators, armed with their lust of being bestowed community recognition of their self-imposed adjudicative powers of stripping the rights of due process from "the poors"m the Foster Care Review Board has a nomination process.

Foster Care Review Board Program

Child Welfare Awards

Each year the Foster Care Review Board provides a forum for recognition of the excellent work that is performed by child welfare professionals who work diligently to overcome systemic barriers and go above and beyond policy, court rule, and statutory standards to help ensure the safety, well being, and timely permanency for children and families served by our foster care system.

TRANSLATION TO REALITY:  Each year the Foster Care Review Board makes no recommendations to the Michigan Attorney General's Office for suspected violations of law and policy, nor does it make recommendations of civil rights violations or fraud within its child welfare system.

The annual awards are presented to those on the “front lines” who have displayed a commitment to excellence and integrity in serving the needs of our states most vulnerable children and families.  One nominee from each of the following categories – jurist (judge or referee), foster care caseworker, lawyer-guardian ad litem (LGAL), foster parent(s), and parent attorney – will be recognized for their exemplary service to abused or neglected children in the Michigan foster care system.  All other nominees will receive a congratulatory letter from the Foster Care Review Board, informing them of their nomination and thanking them for their service.

TRANSLATION TO REALITY:  The annual awards exclude anyone who has fought on the front lines against the nefarious indolence of administrative buffoons like Steve Yager and Bill Johnson.

Finalists are chosen by a panel of Foster Care Review Board members and award winners are chosen by a panel of child welfare professionals and advocates, including award winners from the past year.  Award recipients are recognized at the FCRB annual training conference in early November.

So, please, if you know of someone who is doing outstanding work in any of these categories, take the time to submit a nomination. This will not only recognize their efforts, but will help to highlight to the public the important and often excellent work that is performed within our state child welfare system.

TRANSLATION TO REALITY:  In this nomination process, there are, but of course, limited categories, from which to choose.

Let us examine these categories.

Nominations for the Child Welfare Awards are accepted each year from June through mid-September.  Criteria descriptions, deadlines, and forms are available at the links below:

TRANSLATION TO REALITY:  Notice that there is no category for community advocate, family member, or even a foster child.

There is no option to nominate anyone who has made substantial efforts to end fraud, waste and abuse of Medicaid funding and the horrors of privatization.

There is not even an option for a birth parent who has successfully advocated for their own child.

Why, one may ask.  Well, it would be quite an awkward situation to acknowledge the child welfare system has flaws due to the fact the state is still under federal monitoring.

So, it is at this point I ask for everyone who cares about providing equal justice to the poor, or just to give a voice to a child who is in the foster care system screaming, silently, writhing in seering physical and emotional pain, to give recognition to the work of an individual who has dedicated her life to illuminating the injustices of foster care.

Therefore, it is with the greatest of pleasure that I nominate Debbie Williams, for her work with Families and Children's Justice.

Debbie Williams travels the country, building coalitions for the purpose of forcing "the elected ones" to understand that the time has come to open public discussions on how we treat the poor and their children who cannot afford access to justice.

To nominate Debbie Williams for the Michigan Foster Care Review Board Child Welfare Advocate of the Year, please contact:

Kathy Lohr
Administrative Assistant, Foster Care Review Board
313-972-3288

Remember, there are no civil rights in child welfare.  So, when Kathy tells you there is no category to nominate a community child welfare advocate, remember, ask her why, then let me know what she says.

Oh, and make sure you tell Jim Novell that Beverly Tran is still on a mission...

Michigan Child Welfare Is About To Be Riddled With Lawsuits

Here are a few things which were omitted from the op-ed::

  • For the first time in the history of Michigan, an individual can petition to be removed off the registry after 10 years.
  • It took a class action lawsuit to amend the law to provide due process
  • There are over 100,000 on the registry who were never notified
  • Political officials did not champion the civil rights violations because the contracted CPS entities contributed heavily to the election campaigns of these politicians
  • Individuals who were placed on the registry most of the time, never violated any law nor had a child abuse case.  This was done intentionally for the sole purpose of fraudulently billing Social Security programs such as Title IV-E and Medicaid to the tune of billions of dollars.
  • Children who were improperly put in foster care were drugged, raped, beaten, tortured and murdered which denied removal from the registry because the children could return to the family and initiate legal proceedings against the state
  • Due to the pervasive fraud, many petitions will end up in court.
Get ready, Madame Corrigan and Steve Yager.  I am coming and I will not be alone.

Some time between the births of Mary’s two children, she slipped into the snares of heroin abuse. Fortunately, her mother and stepfather welcomed the three of them into their home in hopes that they could care for the children while she got the help she needed. When the situation instead deteriorated, Child Protective Services was forced to intervene. It conducted an investigation and had Mary removed from her children.
The obvious temporary solution was to have the little ones remain with their grandmother, right? Wrong. When CPS did its investigations, it found, to the grandmother’s horror, that her name appeared in the State of Michigan Central Registry for Child Abuse and Neglect. This was apparently due to an administrative confusion with a perpetrator whose name and address were similar.
Over the next several weeks, the confusion was cleared up and Mary’s mother’s name was duly expunged from the registry. The problem, however, was that CPS had to make an immediate emergency decision regarding the children’s temporary placement. As long as the grandmother’s name was on the registry, she was legally ineligible to have them in her home. And by the time of the expungement, the children were already getting accustomed to their new foster home. At this point, CPS judged, perhaps understandably, that it would not be in the children’s best interests to move them again — even back to what had been their real home.
Like every other state in the nation, Michigan has a procedure for maintaining records of child abuse and neglect. Like the vast majority of other states, Michigan authorizes the use of a statewide central child abuse registry. It is a critical component of the Michigan Child Protection Law, the goals of which include protecting children from abuse and providing supportive services to families. Accordingly, the law mandates the reporting — and recording — of individuals suspected of neglecting or abusing children.
The benefit of this system should be clear, at least in its intention. Indeed, it must be agreed that schools, churches, day care providers, and organizations overseeing foster care and adoption, must have a means to screen out candidates who might be unsafe with children.
The problem is that the recording mechanism has effectively served as a dragnet, entangling individuals whose actions were unreliably reported (possibly by mistake, or else, as part of unseemly blackmail campaigns) alongside with actual abusers. This is so because of two ill-crafted features of the law.
One is that the criteria for registry eligibility is established solely by CPS categorizations. The result is that upon completion of an internal investigation of a report of abuse or neglect — and possibly without any consultation with the police or legal community — CPS often decides that, although its findings do meet certain departmental criteria, the case lacks merit to go forward. Nonetheless, the individual’s name may be placed on the registry and remain there for the rest of his or her life.
The other is that CPS is not required to notify the individual. While it’s true that in egregious cases (as when the individual is made subject to official criminal or child protective court actions), he or she may be well aware of his or her placement on the registry, in many others, there is no notification. While the law does establish an administrative process for expungement from the registry, this is obviously a useless remedy if you don’t know your name is on it in the first place. By the time you are notified, either by a humiliating termination from your employment or, as in the case of Mary’s mother, the nightmarish family separation, it will likely be far too late to repair the damage.
Renowned Jackson-based child welfare lawyer Elizabeth Warner got it right when she stated that the child abuse registry is the state’s “biggest civil rights violation.” One suspects that the reason this has been allowed to stand unchallenged is political. After all, the community of alleged child abusers is not exactly a constituency whose rights public officials are rushing to champion.
This is why we all should celebrate Public Act 30 2014. Effective Sept. 7, this law will require that individuals placed on the registry be notified via registered or certified mail with explanations both of the reason they are being placed and the nature of the appeal process. Reasonably, it also sets a time limit within which an individual so notified must exercise his or her right to request an expungement hearing. Absent a showing of “good cause,” he or she must file this request no later than 180 days after having been notified.
It is disappointing that the new law doesn’t also address the other major flaw in the registry: that the individual’s name can still be listed by virtue of CPS investigation alone. These authors find this to an unnecessarily prophylactic protection of children at the expense of the civil rights of the individuals accused. While we urge lawmakers to enact legislation that respects these civil rights without compromising child protection, we also encourage readers to support those legislators who voted in favor of Public Act 30.
It’s certainly a step in the right direction.

Michigan Audit Finds State May Face Federal Sanctions in Child Welfare Programs

"Children's Deputy Steve Yager
busted stealing grant funds from kids, again.
Michigan Auditor General has caught child welfare administrators with their hands in the cookie jar, once again; the state is non-compliant in its child welfare programs.  Findings can be found starting at page 150. or here at page 247.

There have been major improvements in the operations of Department of Human Services; unfortunately, it looks like the child welfare portion of it is up to business as usual.

Even though the Auditor General completed his audits through sampling methods, it still verifies what I have been spouting for years.  Everyone in child welfare administration needs to go and that includes Maura Corrigan and especially Steve Yager who is the main man to generate corrective actions to the audit findings.  He is an expert in this as he has been lying about the child welfare operations in the state for decades.  He needs to go not because I personally despise the man but because he lies.

Watch Steve lie:

For the Foster Care and Juvenile Justice programs, Michigan runs the risk of loosing a portion of its federal funding.  In a nutshell, foster care youth who were in Child Care Institution (CCI), just a fancy term for youth lock up facility, had their medical paid with Title IV-E funds.

Title IV-E funds are for clothes, shelter and care of the child, or in this case youth.  Medical care for a ward of the state is paid through... let's say this all together now, Medicaid.

That's right, Medicaid payments were made with foster care funds, but wait, it gets better.  The audit states that DHS "overpaid" for the medical and maintenance for foster youth in juvenile justice.  It seems juvenile justice claimed cost reimbursements via Medicaid.  But wait, it still gets better.

Then Michigan used a portion of the state-funded overpayments to match federal funds.

Of course there is more.  DHS recorded expenditures for the Foster Care Title IV-E Program in the wrong fiscal year; therefore expenditures were overstated for fiscal year 2010-2011 and understated for fiscal year 2011-2012.  In addition DHS incorrectly reduced Social Service Block Grants in fiscal year 2010-2011.

The Adoption Assistance Program had, and has problems.  DHS was suppose to go back to adoptions before 2009 (what I refer to as the Dark Ages of state child welfare) and make redeterminations.  Even though the audit was not specific on the types of redetermination reviews, I am well aware of many families who have adopted what people call "minority children" and "special needs children" who were denied eligibility to adoption assistance.  Denials were based on the mentality of administrators that the population of "minority or disabled" children did not deserve the assistance.

Finally, for its grant of the Chaffee Foster Care Independence Program, DHS paid employees while they were working on non- Youth In Transition contracts.  The audit does not go into detail as to what the other contractual activities were but I may speculate that there may have been questionable payroll charges for those other programs, nothing short of double-billing.

Naughty, naughty Michigan.  It looks like you got busted again for Medicaid fraud in child welfare.
